We provide Support Brokerage services to children and adults participating in OPWDD Self-Direction. Our role is to educate, guide, and empower participants and their designees to successfully develop, implement, and manage their Self-Direction budgets while working toward person-centered goals.
We explain the Self-Direction model in clear, accessible language, including participant rights and responsibilities, available services, safeguards, and the roles of the Support Broker, Fiscal Intermediary, Care Manager, and Circle of Support.
We assist with the development of initial Self-Direction budgets and ongoing revisions, including identifying appropriate services, supports, and safeguards aligned with the Life Plan and valued outcomes.
We support participants and designees with understanding and using their approved budgets, monitoring expenditures, reviewing reports, and identifying when amendments or adjustments may be needed.
We collaborate with Care Managers, Fiscal Intermediaries, providers, and families to support continuity of services, address concerns, and ensure Self-Direction services remain person-centered and compliant.
